How do I know if my doctor is part of this network or not?
Ask your doctor's office if they accept the network provider listed on the back of your card. Or call (888) 350-1500.
Do I have a co-pay when going to see a doctor?
No, you pay nothing at the time of service. You are responsible for the balance AFTER the network repricing are completed.
Can I get my lab and x-rays done at the doctor's office?
Yes you can but you need to call (866) 763-3837 to find out if they're part of the network first.
How do the dental, vision, hearing and chiropractor benefits work?
Dental, vision, hearing and chiropractor benefits are all discounts at the time of service.
Do I receive a free eye exam per year?
Yes, you receive 1 free eye exam per year and discounts on lenses, contacts and frames.
Can I go to any hospital or emergency room that I want?
Yes you can, however if you stay within the your network you will receive a much better discount.
Do I need a referral to go see a specialist?
No, you can go to any doctor that you would like in the network.
Can I have elective surgeries on this plan?
Yes, you will receive a re-pricing through your network and the Karis group if the procedure is over $2500.
If I cannot pay my medical bills, can you help me?
Yes, we are contracted with 2 finance companies who will help you pay the bill if you qualify.
Will I get a refund if it is not what I expected?
Yes, we will offer you a full refund if it is not what you expected. Benefits USA offers our clients a 10-day Free Look period. Once you receive your membership packets, it is your responsibility to review all the materials.
After this 10-Day Free Look period, members may cancel at any time by providing written notice at least 10 days prior to their next billing date. There is no refund of the one-time enrollment and processing fee.
